What is orthodontics?

Orthodontics is a special branch of dentistry that deals with studying, diagnosing and treatments of orofacial and jaw growth disorders, as well as correction of irregular tooth positions.

Why is this important?

Treatment of improper position of the teeth and of orofacial disorders reduces risk of dental caries, periodontal disease and temporomandibular joint. Also, correction of anomalies will prevent and treat numerous functional problems with breathing, chewing, swallowing and speaking.

In addition, orthodontics helps patients have a nice smile and youthful appearance, all of which has favourable effects on confidence, and the treatment is painless and effective.

At an early age, in childhood, orthodontic appliances help the child to grow out of bad habits such as pushing the teeth with the tongue, breathing through the mouth and thumb sucking.

When to start the treatment?

Commencement of the treatment depends on the person and the gravity of the anomaly. This is why it is best to perform a preventive check-up about the age of five, so that a treatment – if necessary – can be properly planned.

List of treatments:

  1. “Invisible” appliances (eClinger, lingual braces)
  2. Fixed orthodontic appliances (standard, Alexander brackets, aesthetic , ceramic brackets, Damon system)
  3. Removable appliances (children appliances, Inman aligner)
  4. Retainers (fixation of the teeth after a treatment of fixed orthodontic appliance)
  5. Myofunctional appliances

eClinger or Removable clear occlusal splints for restoring teeth in their correct position

These techniques are applied in corrections of smaller irregularities in adult patients who wish to restore the correct position of their teeth in a discrete manner.

An innovative and very well documented study has shown that eClinger is one of the leading aligner systems. Numerous advantages ultimately lead to the most important outcome– a satisfied patient.

Advantages and reasons to choose eClinger are:

  • innovative technology based on extensive experience;
  • starting and final solutions may be seen even before the commencement of therapy;
  • aesthetically satisfactory;
  • the thinnest and least conspicuous aligner system in the market;
  • digital making enables precise correction;
  • highly pleasant to wear due to its extremely soft plastic materials;
  • does not impede speech;
  • a quick option in treatment planning;
  • comfortable, inconspicuous and painless;
  • can be removed at any given moment;
  • it requires only a few brief check-ups;
  • no limitations with respect to oral hygiene.

Lingual braces

Hidden brackets, such as lingual braces, are placed behind the teeth, thus enabling dental straightening treatment to be completely “invisible”. This way the treatment has no negative effect on the patient’s social life.

Whereas standard brackets are fixed onto the frontal surface of the teeth, lingual braces are practically invisible because they are placed onto the interior (lingual) surface of the teeth. They are suitable for patients of all ages, and they are ideal for those who want their teeth straightened, without the braces showing.

Placement procedure is the same as with other fixed braces. Check-ups are scheduled for every 4 to 6 weeks. Duration of the treatment is between 6 months and 2 years. After the completion of therapy, retainer is mandatory and it is usually advisable that it should be a fixed retainer.

Removable braces

aligner

Removable appliances used to be principal means of treatment. Nowadays they are usually used for mixed dentition stage (8 – 9 years of age). Removable braces consist of an acrylic base and wires bent to form different shapes. The wires are connected to the base, and the base usually has some kind of screw that helps it expand and produce orthodontic force. Different constructions of the base and screw enable proper action to be applied on the teeth.

The patients themselves can remove and place the removable appliances, and the orthodontic forces act as long as the patient wears the device in the mouth. It is usually prescribed that the patient wears such appliance during the night and during the time spent at home. During different activities involving social interaction (school, hobbies etc.) it is not customary to wear the appliance, so as to avoid speech and aesthetic disturbances, which is of crucial importance for patients who are in puberty or adolescence. Removal of the appliance also enables proper oral hygiene maintenance, which can be rather demanding when it comes to fixed appliances. A major fault in all removable appliances is the very possibility of their removal, given that the patients themselves fully control the amount of time spent in wearing such devices. Namely, patients often do not wear the appliance long enough, or because of possible inconveniences they completely avoid wearing the appliance. This is why the success of this kind of treatment completely depends on self-discipline and conscientiousness of the patient.

MND Dental usually makes Twin-block and Inman Aligner removable appliances.

Twin block

It is ideal for children with mixed dentition who suffer from crowded dental arches, irregular position of the teeth and orofacial irregularities according to Angle’s class II or class III. Apart from readjusting the position of the teeth and expansion of the jaw, this appliance may simultaneously rectify the functional relationship between the upper and lower jaw. This would shorten the duration of the treatment and avoid any potential need for designing additional appliances to achieve all of these results.

Inman Aligner

Inman Aligner braces are suitable for anyone who wants to correct the position of their incision and canine teeth, with less crowded dental arches, of 3 and not more than 5.5 mm. It is also necessary that lateral teeth, as well as the ones that are to undergo this treatment, are completely developed and erupted. This device also provides an ideal treatment before cosmetic restorations such as lumineers. Once placed, Inman Aligner method guarantees success which further depends on the patients – regular check-ups and oral hygiene which is not in the least complicated.

Fixed braces

fiksneproteze

Fixed braces consists of metal and ceramic brackets which are attached to the front surface of the teeth, to each separate tooth, to be more specific, enabling improved control over the shifting of the teeth. The metal wire, known as the archwire, is attached to every bracket and it moves the teeth, correcting their position and malocclusion.

Fixed braces may solve any orthodontic and dental problems of patients of all ages, which means that they are absolutely adjustable.

At MND Dental Office we work with fixed braces of the following manufacturers:

  • DENTSPLY GAC
  • Forestadent
  • American Orthodontics

What is the procedure like?

For the patients to have exactly the smile they wish, the orthodontics specialist must decide which bracket system to use.

Necessary X-ray scans, photographs of the patient’s smile, i.e. teeth models, need to be done and made during the first appointment.

During the patient’s second appointment, the dentist places the brackets. Further check-ups are scheduled for every 4-6 weeks, and they will take approximately 20 minutes, with the aim of adapting the force required for the teeth to align in a row, as well as to check if the treatment is progressing in the desired manner. Once the treatment is over, a retainer whose function is to ensure proper position of the teeth is placed, and the patients will then be able to enjoy their new smile.

We would also like to mention that the modern models of brackets are much more attractive than they used to be.

What does it feel like after completing a treatment including fixed braces?

Between the first placement of braces and the following check-up appointment, it is possible to experience some sense of tension in the mouth, given the fact that the teeth will start to shift. This feeling is temporary and after a few days the mouth will already have adapted to the presence of brackets.

By correction of the position of the teeth, speech will be significantly improved, as well.

Brackets are fixed onto the front surface of the teeth and cannot be removed during brushing. This is why oral hygiene is extremely important. MND Dental provides the patients with an orthodontic kit with special interdental brushes which help the teeth and brackets to be clean at all times. The only problem with this procedure is the fact that, after fixing the brackets, certain food stuffs must not be consumed, because there is a possibility of displacing the braces.

This is why, for the purpose of preserving the health of teeth and of the brackets, it is best to avoid:

  • Caramels, chewing gums and chocolate;
  • Soft drinks and fruit juices;
  • Solid food, which may cause damage to the brackets (bread crust, apple bites);

How long does the treatment last?

Duration of the fixed braces treatment is an individual matter. It usually takes from 6 to 24 months to complete it, but this may vary, depending on individual needs of patients during the treatment, as well as on the complexity of the condition.

Check-up appointments are usually scheduled every 4 to 6 weeks.

Retainer

retainer

Retainer is an orthodontic custom-made appliance, designed to hold teeth in position after realigning the teeth by means of fixed braces. Namely, once the brackets are removed, teeth tend to settle back into their original position. Retainers prevent this from happening and they maintain achieved results of the successful treatment.

Depending on the patient’s lifestyle and way of life, dentists recommend fixed or removable retainers. Fixed retainer is a thin wire shaped in a way that it is attached (bonded) directly to the backsides of the teeth. It is fixed with composite material and permanently keeps the teeth in the desired position. Removable retainer is made of transparent plastics and the patients themselves may remove it. It goes round the entire jaw.

Myofunctional Trainer

trainer

Trainer encourages patients to chew properly by using their jaw muscles so that they will adopt proper position of the tongue during swallowing, but it also motivates them to breathe properly through the nose and similar. Research has shown that the manner in which these functions are performed affects proper jaw growth and development.

Infant Trainer may be used in prevention of orthodontic treatments at an older age.

It is used with children between the age of 3 and 5.

There are also trainers for primary (milky) dentition stage, early and late mixed dentition stages, as well as special trainers which are used in fixed braces treatments.

All trainers help the patients develop proper relationships between the upper and lower jaw, i.e. to find the least aggressive way of proper muscle functioning.
